Records for another provider

Ask the provider to: 

• Call Health Information Management at 740-454-5905 (Option #2), or
• Fax request to 740-454-5584
  • Records will be sent directly to the provider at no cost.

Required forms to complete: 

• Medical Records Authorization Form: For general records, including heart procedures (echocardiograms)
• Part 2 Consent Form: For substance use disorder records

Forms are available from your nurse or can be printed from the Genesis website.

Ensure the form is filled out, dated and signed to avoid delays.

Identification needed: 

Bring a driver's license or government ID. If requesting someone else's records, include legal documentation (Examples: Power of Attorney, guardianship, executor or a signed release).
