Credentialing & Licensing
We take pride in the physicians on our medical staff. To make sure quality physicians join us, we have a thorough credentialing and licensing process, which is explained here.
Before we send you an application packet, you must meet the following criteria:
- Current Ohio license or evidence that an application for an Ohio license has been initiated (http://www.med.ohio.gov/)
- Satisfactory completion of an approved postgraduate training program or proof you are in the last year of a training program
- Current clinical competency evidenced by hospital staff membership and inpatient hospital practice within the past 24 months or completed postgraduate training within the past 24 months
- Ability to participate in federal health care programs
If you meet the above criteria, you may submit a CAQH application. If you currently have a CAQH application, you should indicate accessibility to Genesis, as well as to ensure the re-attest is current.
In addition, you will need to contact the Medical Staff Office for additional Genesis-specific documents and to pursue privileges on the Genesis medical staff.
You must also sign a consent for release, which authorizes the Medical Staff Office to consult with members of other medical staffs, education, training, malpractices and professional references with whom you have been affiliated. We require the primary source for all verifications.
With the completion of your file, all documentation listed has been received, the file will be presented to the appropriate department chair for review/recommendation. Your file will then be presented for review by the Credentials Committee at the next regularly scheduled meeting (first Wednesday of each month).
The file with the Credentials Committee’s report will be submitted to the Medical Executive Committee (second Wednesday of each month) for review/recommendation. After recommendation by the Medical Executive Committee, your file will be forwarded to the Board of Directors (first Tuesday of the following month) for approval of privileges.
