You may be contacted after your visit by Professional Research Consultants (PRC) to share your experience by phone or online. Your responses are private and combined with others to help us improve care for all patients.

When will I be contacted?
Not all patients are selected. If you are, PRC may contact you shortly after your visit.
Is my private information shared?
Your medical records stay private. PRC only receives contact details and basic visit information.
Do I have to participate?
No, you do not have to participate. Your feedback is helpful, but you can choose not to take part at any time.
