Genesis has prepared the 2025 ACA 1095 forms for all current and former employees who were benefits-eligible during the 2025 calendar year.

Current employees

How to Access and Print Your 2025 ACA 1095‑C Form in Workday:

1. Log in to Workday.
2. From the left menu, hover over Personal.
3. Select Pay/Benefits.
4. Click Benefits.
5. Select ACA 1095 Forms.
6. Open your form and select print if you need a paper copy.

If you are unable to print, you may request a copy of your 2025 ACA 1095 form by emailing: Benefits1@genesishcs.org.   

Former (termed) employees

If you are no longer employed with Genesis, you may request a copy of your 2025 ACA 1095 form by emailing:  Benefits1@genesishcs.org

Please include:

• Your full name (at time of employment)
• Last four digits of your SSN
• Current mailing address

Genesis has up to 30 days from the date of the request to provide the form.
