The Genesis Primary Care Connection Clinic is for patients who don’t have a PCP and need follow-up care after being in the hospital or the Emergency Department. The staff ensures you have a smooth and stable transition from the hospital to home and will connect you with a PCP who can manage your care.

Also, if you can’t get into your PCP for a same-day appointment, the Primary Care Connection Clinic can be used for acute symptoms. 

Reasons to visit may include:
• Services for hospital discharge follow-up appointments
• Cold/flu symptoms
• Rash/itching
• Urinary tract infections
• Sinus, ear or eye infections
