The staff ensures you have a smooth and stable transition from the hospital to home and will connect you with a PCP who can manage your care.</p><p class="p1">Also, if you can’t get into your PCP for a same-day appointment, the Primary Care Connection Clinic can be used for acute symptoms. </p><p class="p1">Reasons to visit may include:<br>• Services for hospital discharge follow-up appointments<br>• Cold/flu symptoms<br>• Rash/itching<br>• Urinary tract infections<br>• Sinus, ear or eye infections</p>
