Medical Records

We understand obtaining copies of your medical records is very important to you. Genesis has contracted with CIOX Health (an outside agency) to process all medical record requests. There are a few steps that you will need to follow before your request can be processed. Following these steps will protect your privacy and guarantee that the appropriate information is released.

If you wish to obtain a copy of your medical records, a Medical Authorization Form must be completed. You can obtain the form by asking your nurse for a copy or you can print one off from our website, genesishcs.org, after discharge. Delays in processing your request may occur if the authorization is not fully completed, dated and signed.

Once the authorization form is received and verified to be HIPAA compliant, records will be processed and mailed from CIOX Health staff from its corporate office in Alpharetta Georgia.  Upon receipt of records, an invoice will be included for payment pursuant to the below fee schedule and Ohio Revised Code. Please mail payment to the CIOX Health address listed on the invoice. For your convenience you can fax your request to (740) 454-5057 or mail it to 2951 Maple Ave, Zanesville, OH 43701, Attn: Medical Records. Records can also be obtained directly from the medical records department located at 379 Adair Avenue, Zanesville, Ohio 43701. You will need to show identification (for example: driver’s license or government issued ID) when requesting records or yourself. If requesting another individual’s records, you will need to show legal documents proving your authorization to receive these (e.g. a power of attorney, guardianship, executor of the estate, etc) or have a signed release form (release only applicable for patient’s 18 years of age or older and able to give such release).

All diagnostic imaging studies performed are kept in the Radiology department. If you need copies of imaging reports and films on CD, you will need to contact the Radiology department at 740-454-4009.

Upon receipt of the valid HIPAA-compliant authorization, standard processing is 5 business days but up to 60 days for sizeable or archived (off-site) records. Please note that you will be charged for medical record copies if requesting for personal use, for legal purposes, and most other non-treatment purposes unless otherwise required by Ohio law (i.e. first copy is free of charge for BWC, Industrial Commission, Department of Job and Family Services, and to support a claim of Social Security Disability or Supplemental Security Income (SSI) and the request is accompanied by documentation that a claim has been filed). Records being sent directly to a physician office or other health care facility for treatment purposes will be provided at no charge as a courtesy to you.
