Medical Records

Your medical records are important to you, and we understand that. We want to help you get a copy of your medical records, and there are a few steps you'll need to follow. These steps are important so we can protect your privacy and make sure only the appropriate people can have access to your records.

If you want a copy of your medical records, please completely fill out the Medical Records Authorization Form. If the release isn't fully completed, dated and signed, you might not get your copy of the records as quickly. 

Once your request is received and verified by Genesis health information managment staff, Health Port (which is an outside agency) will copy and process the medical records.

Health Port will then send medical record copies along with an invoice for those requests when the total number of pages in the records is 100 pages or less. If your request is for records with more than 100 pages, Health Port will send a fee approval letter that will include payment options and total fees for the medical record copies.

You will then need to send your payment for the requested records to the corporate office in Alpharetta, Ga. The address will be on the fee approval notice. (Please note that delay in payment will cause a delay in copying of the medical record.)

When the corporate office receives payment, Health Port (onsite at Genesis) will copy and mail the records to the address provided on the authorization. The records should be received approximately 7-10 business days following the receipt of payment.

The fees for copies of medical records have increased in Ohio effective Jan. 1, 2011. Per Ohio Revised Code 3701.742, the Ohio Department of Health is required to adjust the fees that health care providers and medical records companies may charge for medical records during 2011.
