Reducing Elective Deliveries - Improving Patient Care

Genesis recently completed a project using Mom Dad BabyCrimson, the Performance Management Initiative, to assess re-admission rates for newborn babies who are delivered prior to 39 weeks gestation, many of them appearing to be elective. Our re-admission rates were higher than many other of the 350 hospitals in the Crimson system with Level II nursery care. OB studies have shown that elective deliveries before 39 weeks increase the risk of newborn respiratory distress as well as increase the rates of C-sections where there is a higher rate of postpartum anemia and longer lengths-of-stay for both mothers and babies.

A team of physicians and staff worked on a Crimson project to determine how the reducing of elective pre-39 week deliveries might affect the re-admission and length-of-stay rates. The results were impressive.

An overall 8 percent decrease in elective pre-39 week deliveries impacted our re-admission rates for infants dramatically. Our NICU admissions decreased 25 percent; length-of-stay for newborns decreased from 2.85 to 2.65; and the overall pediatric re-admission rates decreased from 2.79 to 1.84 percent in a 12 month period. Charges were decreased by $300 per newborn, equating to an annual decrease of $426,600.

The impact on the OB unit was significant as well, with a decrease in complications for mothers from 1.17 to 0.42 percent and a decrease in re-admissions of mothers from 2.44 to 1.79 percent.
