Genesis is pleased
to provide members of the clergy and lay representatives the
opportunity to visit patients while they are in our hospital.
We encourage you to apply for a Genesis photo identification (ID)
badge. This will assist our patients and staff in clearly
identifying you when you are in the hospital. While you may visit
patients during regular visiting
hours without an ID badge; you must have an ID badge to enter
the hospital after regular visiting hours.
Once you receive your Genesis photo ID badge, you will also
receive the following benefits:
- A 15% discount on meals in either of the hospital’s
cafeterias.
- A free training program on hospital visitation for lay
representatives.
- Notification of upcoming continuing education seminars for
clergy sponsored by the spiritual care department at Genesis.
- Annual clergy breakfast.
The ID application process costs $20. Follow these steps:
- Print the application
form and complete it.
- Print the confidentiality
statement form and complete it.
- Mail both documents along with the $20
fee (made payable to Genesis HealthCare System with “Human
Resources” written in the memo section of the check) to:

A background check will be completed by Genesis, and once
approved, you will be notified by the spiritual care department to
stop by the human resources department to have your photo taken
for the badge.
