You will need to completely fill out an authorization
form
to obtain your medical record information. If the
release is not fully completed, dated and signed, the copying of
your records will be delayed.
Once your request is received and verified by
Health Information Managment staff, Health Port (who is an outside agency) is responsible
for copying and processing your records. Their procedure is
as follows:
a. Health Port will send you a fee approval
letter with the total charge listed for copying your
records
b. You will then
be responsible for sending payment for your records to the
corporate office in Alpharetta, GA. The address will be on the fee approval
notice. (Please note delay in payment will cause a delay in
copying your records.)
c. Upon receipt of payment to the corporate
office, Health Port
(onsite at Genesis) will copy and mail
your records to the address provided on your authorization. You should receive your
records approximately 7-10 business days following the receipt of
payment.

The fees for copies of Medical Records have
increased in Ohio effective January 31, 2007. Per Ohio Revised Code
3701.742, the Ohio Department of Health is required to adjust the
fees that health care providers and medical records companies may
charge for medical records during 2007. For more information
